Antibiotics are not simply overused; they are misallocated. A framework that estimates how much of each type of drug 186 countries actually need finds near-universal overuse of broad-spectrum medicines alongside a shortage of the last-resort ones

【1】Modern medicine rests on a quiet assumption: that a routine infection can be cured with a cheap pill, and that surgery and childbirth will stay survivable because of it.That assumption is eroding, since bacterial resistance was associated with 4.71 million deaths in 2021 and directly responsible for 1.14 million of them.
【2】The reflex response is a slogan, prescribe fewer antibiotics, but the arithmetic behind it is muddier than it sounds. The world is not using too much of one thing; it is using too much of the wrong antibiotics and too little of the right ones, a double failure a July study set out to measure.
【3】The work was led by Aislinn Cook of City St George's, University of London, with colleagues at Oxford and funding from the Wellcome Trust.Rather than counting how many antibiotics countries consume, the team asked how many they would need if every bacterial infection were treated properly, across 186 countries holding 99.8 per cent of the world's population.
【4】Its yardstick was the World Health Organization's AWaRe ladder, which sorts antibiotics into Access drugs for common infections, Watch drugs that are broader and more powerful, and Reserve drugs kept as a last line.Countries were sorted into peer groups sharing infection burdens, resistance patterns, income and health-system capacity, and within each group those using least and losing fewest patients became the benchmark.
【5】On that basis the authors estimated that the world needed roughly 43 billion days of antibiotic therapy in 2019, about one course per person per year, of which 77 per cent should come from the Access tier. That lends quantitative support to the target agreed at the United Nations in 2024, which asks that 70 per cent of antibiotic use be Access drugs by 2030.
【6】The comparison with reality is where the argument sharpens.Among 67 countries with usable consumption data, 72 per cent used more antibiotics in total than their infection burden justified, 99 per cent overused Watch drugs, and 60 per cent still prescribed medicines the WHO calls unfit for routine use.Meanwhile 42 per cent used too few Access drugs and 52 per cent too little Reserve, so patients with the hardest infections may be denied the drugs most likely to save them.
【7】There is an inversion at the heart of the picture.Rich countries consume by far the most Watch and Reserve antibiotics, yet more than 80 per cent of the world's optimal need for them sits in poorer countries, where infection and resistance burdens are heaviest.
【8】Because primary care accounts for about 90 per cent of human antibiotic use, stewardship must pull in two directions at once.Overprescribing of broad-spectrum drugs has to fall where a narrow one would do, while supply of quality-assured Access and Reserve drugs has to rise, which means procurement, affordability and locally set targets rather than global exhortation.
【9】None of this will feel dramatic.Resistance has no single outbreak to photograph, only a slow drift in the share of infections that refuse to clear, and its bill is settled in ordinary wards. The study's contribution is to move the question from how much the world consumes to how well that consumption fits the disease it is meant to treat.

Modern medicine rests on a quiet assumption: that a routine infection can be cured with a cheap pill, and that surgery and childbirth will stay survivable because of it.
主干:Modern medicine rests on a quiet assumption(主语 + 谓语 + 介词宾语)
并列结构:that a routine infection can be cured ..., and that surgery and childbirth will stay survivable ...(两个 that 都不能省,因为它们引导的是同位语而非宾语)
The world is not using too much of one thing; it is using too much of the wrong antibiotics and too little of the right ones, a double failure a July study set out to measure.
too much of / too little of:两个量词短语对举,把“过量”与“不足”并置在同一句里
同位语:a double failure(概括前面“错的多、对得少”),后接省略关系词的定语从句 (that) a July study set out to measure
Countries were sorted into peer groups sharing infection burdens, resistance patterns, income and health-system capacity, and within each group those using least and losing fewest patients became the benchmark.
主干:两个并列分句 —— Countries were sorted into peer groups(被动), those ... became the benchmark(主动)
代词 those + 分词:those using least and losing fewest patients(“用量最少、失去病人最少的那批国家”)
Among 67 countries with usable consumption data, 72 per cent used more antibiotics in total than their infection burden justified, 99 per cent overused Watch drugs, and 60 per cent still prescribed medicines the WHO calls unfit for routine use.
Among 67 countries with usable consumption data
比较结构:more ... than their infection burden justified(“超出其感染负担所能证明的合理量”,than 后是省略了主语的从句)
省略关系词的定语从句:medicines (that) the WHO calls unfit for routine use(call sth adj.
Overprescribing of broad-spectrum drugs has to fall where a narrow one would do, while supply of quality-assured Access and Reserve drugs has to rise, which means procurement, affordability and locally set targets rather than global exhortation.
主干:Overprescribing ... has to fall ..., while supply ... has to rise(while 表对比,两个主语一降一升)
where 引导地点/情形状语从句:where a narrow one would do(“在窄谱药就够用的地方”,one 代指 drug)
rather than:procurement, affordability and locally set targets rather than global exhortation(肯定前者、否定后者)
仿写题:仿照第 9 段 "The study's contribution is to move the question from how much X to how well X fits Y."(把问题从“数量”转向“匹配度”)的句式,就一个你熟悉的公共议题(如作业量、屏幕时间、城市绿地)写一句“重新定义问题”的评价。
